Volunteer Certified Application Counselors from the Oakland Livingston Human Service Agency will help uninsured persons under age 65 navigate the healthcare marketplace and enroll in a plan. Appointments are required - call 248.524.3484. Allow two hours for your appointment. You must bring to your appointment social security numbers and employer and income information (pay stubs or W-2s for example) for everyone in your household who needs coverage, policy numbers for current health insurance plans, and a completed Employer Coverage Tool for every job based plan you are eligible for (even if you are not enrolled.). Your employer should have this form.
